Group Plan 


 You are entitled to the preferred benefits that have been purchased and added to your group vision plan. They may include any or all of the following:
enriched benefit levels of 15% - 25%

extended warranty  that covers all repairable damage to the frame and lenses for the length of the benefit period
direct payment of eligible benefits to the practitioner leaving only the uncovered benefit to be paid by the plan user
minimum benefit of $125 towards laser vision correction
benefits toward safety eyewear

The cost of the Plan is dependent upon:

the services you purchase
the number of primary users of those services
