procedures for using Spencer Vision and obtaining your enriched benefits
are easy.

Just show your designated identification card to the
store employee. |

Select your frames, lenses or contact lenses as
provided for in your group or individual plan. |

Complete
and sign the User portion of the claim form, when required.
